It contains best practices regarding the investigation of events to ensure that it is effectively organised and conducted to determine the direct causes, root causes, contributing factors and lessons learned to improve safety and prevent recurrence of events. Use of the guidance provided by this publication will be helpful to nuclear facilities and related institutions to strengthen and enhance their feedback process through the implementation of best practices in the organisation, management and conduct of an effective investigation of events.
